Who is DRIVER B in car insurance terms?

I have to fill out some paper work from my car insurance, and it is asking me to choose between DRIVER A, DRIVER B, OR WITNESS. But I dont know the difference of what they mean by DRIVER A, DRIVER B OR WITNESS, so can someone please tell me what the difference

Driver A is you, Driver B is the other vehicle that was involved in the accident. Witness is anyone who saw the accident.

deductible
deductable/waiver

what is the difference? why would 2 choices be offered? thanks for your replies.
